This form is to be used for property and injury liability claims only. Before completing the form please read the following information:

You should only use this form if you wish to claim for an incident that has caused loss or damage to your property, and/or personal injury

  • The acceptance of this form does not imply liability for your claim has been accepted by Havebury or you will automatically receive compensation
  • If you have Home Contents, Buildings or Motor insurance covering your losses, it can be advantageous to submit your claim to your own insurer to handle on your behalf. You will only be compensated under this claim, if Havebury has been negligent
  • Havebury’s insurers are allowed up to 90 days from the date they acknowledge your claim to complete their liability investigations
  • Havebury do not handle liability claims internally. Havebury’s insurer will communicate with you directly and take full ownership of liability and settlement decisions
  • Please complete all relevant sections. Incomplete forms will delay the processing of a claim.
