The survey aims to ensure we have the most up-to-date information about you and your families. Not only does it help us tailor our communications and messaging, including the best way and time to contact you, but it also helps us understand your individual needs and requirements and how we can ensure the services we provide meet these.
You should soon receive an email or a letter from us explaining how to complete the survey. If you receive an email but would prefer a paper copy, please let us know by emailing firstname.lastname@example.org or calling the Communications team on 0300 330 900 and enter extension number 7314 when prompted.
It should take about 10 minutes to answer the questions, and we’ll enter you into a prize draw to win supermarket vouchers of your choice. We’ll be giving away:
- 1 x £300 supermarket voucher
- 2 x £100 supermarket vouchers
- 10 x £50 supermarket vouchers
